 Yasser Hammoud, MDY. T. Hammoud, M.D. has served as CEO and Medical Director of UOP since its inception in 2002. Dr. Hammoud's unique clinical experience, skill, and understanding of medicine is highly exceptional. Dr. Hammoud is a Diplomat of American Board of Family Practice. He founded the Ford Tel Medical Center and has 20 years of practice experience. Dr. Hammoud has served as Director of Care Management of a large hospital system. He has served on operating and advisory committees of the largest provider groups and payers in Michigan. He is recognized by senior management in the healthcare industry as a distinguished and significant leader. Under Dr. Hammoud's leadership UOP has grown to one of the largest physician organization in southeast Michigan and has developed a reputation for focusing on quality and efficiency. His approach to healthcare does not waver. -I believe in a Patient First approach to medicine. We will always place the patient first in everything we do. |

| Marya Alazzawi, M.D, M.B.A. Dr. Alazzawi joined UOP in 2005. She serves as Director of Utilization Management and Director of Finance of UOP. In this role she oversees the utilization management program by working with physicians in providing clinical expertise and guidance in order to assist physicians in improving their performance. In addition she monitors in patient continued stays that do not meet established criteria so as to improve facility utilization. Dr. Alazzawi is also involved with the application of quality principles related to patient care processes. In her role as Director of Finance, Dr Alazzawi oversees the revenue performance of UOP as well as the revenue performance of all UOP Physicians participating in managed care health plans. Dr Alazzawi has a master's degree in Business administration from Lawrence Technological University. Previous to serving in her present capacity she was in private practice. |

 Terry Gordon, MDDr. Gordon serves as Associate Medical Director of UOP for Quality and Plan Care Management. His primary responsibilities are to promote the delivery of quality health care while also focusing on the appropriate and effective utilization of plan resources. Prior to serving with UOP, Dr. Gordon was the Medical Director for Aetna U.S. Healthcare for the Michigan Market. Dr. Gordon is a graduate of the University of North Carolina and has completed post graduate training at the Mayo Clinic and the University of Michigan. |

 William RuncoMr. Runco serves as Chief Operating Officer and General Counsel for UOP. In his dual role, Mr. Runco is not only responsible for the operations of the organization, he oversees all legal matters pertaining to UOP and is directly responsible for healthcare contracting matters. Previous to serving in his present capacity, Mr. Runco was a Judge of the 19th District Court for twelve years and a member of the Michigan Legislature for 6 years. Mr. Runco is a graduate of the University of Michigan Law School. |
