How it works.
The patient or family chooses a Primary Care Physician (PCP) that is a Family Doctor, Internist, or Pediatrician with whom they feel comfortable to be their “Medical Home” for all aspects of their care. The patient or family will then use their chosen PCP for every aspect of their care. From preventative and well-visits to visits for chronic conditions like diabetes, asthma, Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), Coronary Artery Disease (CAD), etc., and for emergent, non-life threatening issues.

Why PCMH?
As you may know, our healthcare costs and the health of our nation is out of control! The reason? The majority of our population seeks help at high-cost Hospital Emergency Rooms for non-emergent services or by going to a Specialist unnecessarily. Your lower-cost PCP has all your medical records and knows your history which eliminates the need for duplicate testing. Your PCP can then make a medical decision if a Specialist or Emergency Room intervention is necessary. PCMH is an effective way to lower your out-of-pocket costs and keep the patient or family healthier and well-informed.
